Library Electronic Device Loan Agreement Form Preview

PSCC Libraries Electronic Device Loan Agreement

Borrowers will be asked to complete this agreement in a digital format at the Circulation Desk when checking out an electronic device from the library.

1. Student Responsibility

I agree to accept full responsibility and financial liability for the device(s) and peripherals loaned to me and I am responsible for the replacement costs and processing fees for the/those item(s) if they become lost, stolen, or damaged. (Example: laptop, adapter/power cord, carrying case)

2. Not Returned by Due Date

Students: An unreturned or damaged device will result in a hold on my student records until the replacement costs and processing fees are paid. While there is a hold on my student records I understand:

  • I will not be able to register for classes.
  • I will not be able to request transcripts.
  • I will not be able to obtain a diploma.
  • Unresolved replacement and processing fees are subject to collection agency referral, fees, litigation, and national credit bureau reporting (see below).


If I break this agreement with the college and fail to pay all charges by the end of the semester, the college may exercise any remedy allowed by law, including without notice or demand (except as required by law) the college may hire or pay a third party to collect the debt, including without limitation, the pursuit of litigation. I understand and accept that if I fail to pay my financial obligation to the college or fail to make acceptable payment arrangements the college may refer my account to a collection agency. I further understand I may be responsible for paying the collection agency fee, which may be based on a percentage at a maximum of 33-1/3 percent of my delinquent account, together with all fees and expenses, including reasonable attorney's fees, necessary for the collection of my delinquent account. I understand that my delinquent account may be reported to one or more of the national credit bureaus.

3. PSCC Computer Usage Policy

I agree to comply with Pellissippi State Community College's Computer Usage Guidelines (Policy 08:13:05) when using the device(s).

4. Deletion of Files

I understand that any files saved to the device(s) during the course of its/their use will be deleted upon return to the library. I am responsible for saving files I need to an external source such as a flash drive.

5. Reporting Requirements

Loss, theft, or damage to the device(s) must be reported immediately to a library staff member at the library's Circulation Desk.


 

6. Device(s) Not Designated as In-Library Use Only 

Items that are not designated as in-library use only may be removed from campus.


  • 7-day or 1-month loan: Device(s) designated for a 7-day or 1-month loan may have the loans renewed up to 3 times if no one has placed a hold request on the device(s) at the time of each renewal request. When the renewal limit has been reached, the device(s) must be returned to the library, but once it is/they are removed from the student's account and if there are no hold requests on the device(s) at that time, the student can check the device(s) out again and the renewal "clock" will then reset. 
  • Semester Loan: Devices designated for a semester loan are eligible for renewal once students have registered for classes for the next semester (MacBook Pros can only be renewed with the permission of the Media Technology Department's designated approvers.)

7. Device(s) Designated as In-Library Use Only

Device(s) designated as in-library use only is/are loaned to me for up to 4 hours and cannot be removed from the building. I may renew the loan once per day if no one has placed a hold request on the device(s). The device(s) must be returned to the Circulation Desk no later than 15 minutes before closing regardless of what time I checked it out.

8. Faculty/Staff Only

Device(s) loaned to me may be removed from the campus. I may renew the loan(s) up to 3 times if no one has placed a hold request on the device(s). I will be billed for replacement costs and processing fees for an unreturned or damaged device. Unresolved replacement costs and processing fees are subject to collection agency referral. Unpaid balances remaining upon termination of employment will be deducted from my final paycheck.
